Welcome, and thank you for your interest in Primary Cares: A Performance Improvement Initiative for Chronic Antiplatelet Therapy. This site will guide you through each step of the PI initiative, allowing you access to key resources and your PeerPoint site coordinator. If you have not yet registered, please use the button above to do so. If you have already been assigned a username and password, use the "Log In" button to access your resources.

Getting Started


  • Stage A— BASELINE
  ο Complete the online Baseline Assessment
  ο View the PI training webinar
  ο Complete posttest/STAGE A evaluation
  ο Download the Baseline Logs/Tools and submit the logs to Peer•Point via fax at 847-440-8800
  • Stage B—IMPLEMENTATION
  ο After completing Stage A, you will be provided with the Implementation Tools/Logs—will help close practice gaps identified by the Baseline Assessment. You will be required to complete and submit the Implementation Logs to Peer•Point via fax at 847-440-8800.
  • Stage C—CLOSEOUT
  ο At the end of the STAGE B, physicians will be asked to fill out a follow-up evaluation on the initiative website.
